MOVING HELPERS
Any Eugene moving company can get added onto A Great Moving Crew's (AGMC's) List of Eugene Moving Companies for free, just by calling AGMC and informing AGMC of that company's moving business website. However, in order for AGMC to list your moving company on AGMC's listing and booking-help service platform as a company AGMC can help book appointments for, you'd need to fill out and turn into AGMC the following three documents.

3) W9.
This contract, application form, and a link to a printable W9 are shown below.
You can either copy and paste these documents from this web-page, and print your own copy of these papers, or you can pick a copy up from AGMC or possibly from one of the other company's listed with AGMC. You'd then need to fill them out and mail them back to AGMC at:

OPEN MARKET LISTING AND BOOKING HELP CONTRACT FOR MOVING COMPANIES
If you (the undersigned) fill out and turn in this contract to AGMC (A Great Moving Crew), this will result in your (self owned) independent moving company becoming listed among other moving companies on AGMC's open market listing and booking help service, (like Home Advisor, Angie's List, and Movinghelp.com) from which moving customers shop for and book the services of independent moving companies. You can define and update your moving company's availability schedule, rates, and services offered.
You agree AGMC has the right to use its merit based point scoring system to determine the listing order that moving companies are presented to customers, which is a prime factor in the frequency of your services being chosen by customers. You are OK with the fact that this system could result in a job frequency that could end abruptly and/or range from lots of jobs to none at all, so you will not rely on this platform as your sole source of customer leads.
If and when your moving company's services are chosen for a job appointment, AGMC will pass along to you the customer's job offer information in the form of a text message sent to you from AGMC that describes the customer's desired job start time, appointment details and contact information. You would then need to quickly confirm or decline the customer's offer. Only customer offers that AGMC believes fall within the date, start time, crew size and other job parameters that you have directed AGMC to try to arrange for you will be offered to you. If a set of job offer instructions fall outside the parameters of what you are instructing AGMC to try to help arranged for you, this contract requires you to decline that job offer.
If in a job offer description text from AGMC, your company's name (or your name) is used in conjunction with the term "Lead moving company" or "Lead" (for short), and if you reply by text with your indication of acceptance, that would constitute you contracting to be "The" moving company that is being hired by the moving customer named in that text, with your company taking sole responsibility and liability for all aspects of that moving job, including for damages, hiring and paying your crew, collecting the service bill, being insured, licensed, tickets, taxes, injuries, accidents, thefts, problems, losses and for everything else about that job.
The booking fee you agree to pay AGMC for this booking-help service is $20 per man hour of labor you charge your customer, or $65 total for 2 man 2hr minimum jobs, to be calculated based on a picture of your customer's job invoice you are agreeing to text to AGMC immediately (within ten minutes) after the customer pays for the job, and paid by you to AGMC by Venmo immediately (within ten minutes) of the customer's payment form clearing. However, while any invoice pictures or payments are late past this deadline the booking fee raises to $22/mhr. and further booking help from AGMC can be restricted or cease at AGMC's discretion. Job offers that are requesting you to make a "Bid" would require you to respond with both your offered bid (guaranteed total price amount you'd charge the customer) and your offer of a set booking fee you agree to pay AGMC if your bid is accepted and you are paid by the customer for that job, which should be calculated based on your estimated labor hours of the job applied to your normal booking fee rate.
If you have indicated to AGMC that you would like AGMC to help you find additional freelance movers to help you on your jobs, AGMC can include in the job offer texts names of potentially available moving helpers that you could choose to hire for that one job appointment. You may also decline the suggested helpers, ask for other options, or choose other help entirely. If helpers are named in a job offer text and you reply with a simple confirmation text, this would constitute your agreement to hire the named helpers as freelance independent contractors working for you for that one single job appointment, at your pre-specified pay rates, with you as the hiring party.
You (the undersigned) agree that for any job appointments that AGMC helps arrange for you, AGMC would be acting only as an open market listing and booking help service, and that you (and your company) would not be working for AGMC, nor be operating as a partner, subcontractor, agent or employee of AGMC. By signing this contract you are declaring that you meet the legal qualifications required for providing your own independent moving company service, taking sole responsibility for potential ODOT penalties, worker's comp, and unemployment claims, and you agree to owe AGMC full compensation for these expenses if they are ever charged by authorities to AGMC. You agree to do your own withholding and reporting directly to tax authorities.
